By submitting this application you are affirming that to the best of your knowledge you are in need of assistance to attend the Winter Seminar. In addition, if you are awarded a scholarship, and your employer subsequently pays for all or a portion of your Winter Seminar fee, your scholarship, or a portion thereof, will be awarded to another applicant.

The record kept of your survey responses does not contain any identifying information about you unless a specific question in the survey has asked for this. If you have responded to a survey that used an identifying token to allow you to access the survey, you can rest assured that the identifying token is not kept with your responses.
